A Permanent account has all of the advantages of a Paid one (10 Gb for your images, no advertising, the maximum possible number of userpics, etc), but you never have to pay for it again.

As a rule, the Permanent accounts cannot be bought: they are given out by LiveJournal's administration for special services in LJ development.
